Self and Family Managed Care Program

Self and Family Managed Care is a unique program that allows WRHA/IRHA home care clients and their families to manage their own home care program. This program provides more flexibility and continuity as the client has control of the schedule and care plan that best suits their needs. The program offers money in exchange for home care services which can be used to hire your own private care or a care company. This allows you the freedom to choose who enters your home and when.

You are eligible for the  Self Family Managed Care program if:

  • You are a Manitoba resident.

  • Registered with Manitoba Health.

  • Require health services or assistance with activities of daily living.

  • Require service to remain safely in your home and require more assistance than available from existing supports and community resources.

We Can Help!

Centered Care Inc can assist you in the process of applying for the Self and Family Managed Care program, determining a schedule and finding the attendants(s) who are the best fit when considering the needs as well as personality. We help make the accounting for this program simple by providing our clients with both a personal invoice along with  one to submit to the WRHA.

Our clients express the most important aspect of their care is consistency. The Self and Family Managed Care program allows our clients to gain that continuity as well as ensure that care is received.
